Described as an “invaluable end-of-life resource from a team of expert counsellors” Transitions encompasses the journey through diagnosis, treatment, recurrence, palliative care, and bereavement this book provides a strengths-based approach that will help those who work in hospice and palliative care to guide patients and families through difficult end-of-life experiences, and provide interventions for each of the key transitions that most dying patients and their families face. Weaved throughout are personal reflections and experiences from social workers, counselors, and other team members on using this approach, providing examples of ways to support the whole care team.

The Victoria Hospice Society Board of Directors, staff, and volunteers extends their sincere congratulations to Marney Thompson and Wendy Wainwright on this achievement.
