Advance Care Planning Day

Event Date:
Monday, April 16, 2018

If you have never heard of Advance Care Planning, you are not alone! 8 out of 10 Canadians have never heard of it.

What is Advance Care Planning?

It is a process of reflecting on and communicating your wishes for end-of-life care with your family, friends and health team.

Most of us hope to die peacefully, able to communicate with loved ones until the very end. It doesn’t always happen that way. Making your wishes known now helps those who care about you make the right decisions if you can’t speak for yourself.

Monday, April 16th is National Advance Care Planning Day.

A wealth of information and tools are available at The Speak Up Campaign website, part of a National Advance Care Planning Task Group comprised of individuals representing a spectrum of disciplines, including health care, law, ethics, research and national non-profit organizations.

Check out this fun video outlining the Five Steps of Advance Care Planning.

Quality palliative and end-of-life care for all: Our mission is to enhance the quality of life for those facing life-limiting illness, death and bereavement through patient and family centred care, education, research and advocacy.
