Many people choose to be cared for in their homes, close to family and friends. Therefore, our services are available to patients throughout the community. This is possible through our partnership with Island Health’s Home and Community Care Nursing and Home Support services.
Our Palliative Response Team (PRT) is a 24-hour, on-call service that provides short-term crisis consultation and treatment for problems that might otherwise require stressful admission to a hospital. The PRT is also available to help support death at home.
Our home care also includes trained community volunteers, palliative care physicians, community counsellors, and bereavement services.
Victoria Hospice patients and family members may also be eligible for home visits provided by Island Health. Services can include visits from nurses, physiotherapists, registered dietitians, social workers, and other health care providers. In addition, help with day-to-day tasks like bathing, dressing, and caregiver support can also be provided on a short or long-term basis and may change as clients’ needs change.

Island Health is committed to supporting those facing end-of-life challenges. The partnership that Island Health shares with Victoria Hospice is important in providing community-based palliative care to people in their own homes. Home & Community Care Nurses are usually the first contact for those registering for Victoria Hospice services.
Sooke Hospice
The program of care that Sooke Hospice has developed remains uniquely appropriate to its community’s needs. The team of Home Care Nurses, physicians, volunteers, Community Health Workers, and counsellors work together to assess and fulfill the needs of Sooke patients and family members. Saanich Peninsula Hospital
Saanich Peninsula Hospital has a 10-bed palliative care unit for patients living on the Saanich Peninsula, north of Victoria, who require short-term symptom management or a longer stay of up to three months.
